Not necessarily indicative of foul play. I have two projects like this (https://github.com/smacke/ffsubsync and https://github.com/ipyflow/ipyflow) and I attribute it to not having great developer documentation.

My idea how to improve finding a correct subtitle offset
It is possible to largely avoid this problem of having to sync subtitles when you go to watch something by using software that can automatically find the offset. I made one a while back that embeds the offset and subtitles into the MKV file: https://github.com/iwalton3/media-scripts/tree/master/subtitle-audio-transfer
