Smart DNS VPS for geo unblocking
In my Azure VPS, I tried several GitHub ideas like this https://github.com/ab77/netflix-proxy but it didn't unblock Hulu. I also tried https://github.com/Seji64/SniDust and it almost worked, I managed to log in to Hulu with it but Hulu web did not allow me to play any content.

How does DNSFlex or Smart DNS services work?
https://github.com/ab77/netflix-proxy is a relatively straightforward implementation, like /u/scottmc83 guessed, it's just the matter of resolving proxied address to an IP you control, then in the server you check the SNI and forward the traffic to your remote server (assuming you don't just resolve the address to the chosen remote server)
