Any ISO patches or mods?
That method you mentioned is no longer used. We now create a patch file for each mod which you can use a website patcher (or an offline one) to patch your game ISO. Read u/xDzonbyDx's great comment on the details.

I Finished CTR 808%
Make sure to use our latest release link, instead of the old links on our YouTube videos. The easiest way to play these mods is indeed through an emulator (Duckstation is the best), but you can totally play these in a modded PS1 or PSP as an E-Boot.
