You need a case with a fan. Heat sinks just can't dissipate heat like a fan. This a RPi3 case but I have similar for my RPi4s. My kits came with a mini hdmi cable and a mini hdmi adapter. For the record, i hate the mini hdmi ports. I don't use the heat sinks, don't need them, got a fan! I can overclock (2147) the Rpis and run stressberry and not be close to throttling. The GPIO header is a little bit harder to work with but not impossible, an extender might help.
