I'm Anathema (Nath), Project Manager for Foundry VTT. Many of you may already know me from my presence on our Community Discord Server, my work on A House Divided, or my involvement with a variety of Paizo product conversions. Allow me to direct you to the Simple Worldbuilding System which was designed for EXACTLY that purpose.
Regarding the community playing an active role in development, I believe the dnd5e team does accept pull requests- but you can read more about that at their repository.
I play a lot of Pathfinder 2E, which is also the system that most of the content I develop is for, so I'm very glad it has such a rock-solid developer team.
The Token Variant Art module can handle this (scroll down to Image to User Mappings). You'll probably need to write some macros to change things on the fly though.

The text editor is foremost aimed at WYSIWYG editing, so it will occasionally edit and restructure the HTML you give it in order to support its WYSIWYG editing capabilities. It sounds like what you're looking for is a raw HTML editing mode that does not assume its content will be edited in anything other than HTML. That is something we could add, we have the markdown editor that does this (but for markdown rather than HTML). I've raised an issue for you here.
