Home-row mods - will I get used to it?
I had trouble with HMR as well at the beginning, but since switching to sunaku's fork/PR I've almost eliminated all the issues I had with it. And worth mentioning is urob's timeless HMR config from which I think the fork/PR takes some of its inspiration.

𝑥MK: Use programmable keyboard firmware with any keyboard.
I have a separate system for that as part of this old unreleased project. They're orthogonal so could be used together, but it'd be nicer if they were integrated. If you just want to remap buttons or map buttons to keys you can do that in the OS.

Well, things like Bilateral Combinations are aware of which hand is using the modifier to only apply it to the other side's presses & minimise accidental triggering on same-hand rolls.

What where the most surprisingly usefull/useless qmk features to you?
I'd like to call out that one of the coolest yet underutilized features is Raw HID, which performs bidirectional communication between QMK and the computer. In principle, this could enable some interesting new things, like having the keymap change adaptively based on the focused application or using the keyboard LEDs or OLED screen to indicated computer CPU use, battery, email notifications. The tricky thing is Raw HID requires writing software for the computer side as well. There is some existing work, like Drugantibus/qmk-hid-rgb and BlankSourceCode/qmk-hid-display, but it would be cool to see Raw HID become more widely used. Hopefully the QMK XAP protocol that's under development will make these kinds of things easier.
