For those looking for a lightweight alternative to Uptime Kuma there is Gatus. It's written in Go and it's really really fast. It also has a status page and notifications support.

As for your question - there are various tools that could help do this - easiest route will be using a no-code automation tool + form tool as others have mentioned. Zapier would be easiest but might get expensive fast since they have per record, per step pricing (I think same thing with Integromat). n8n is a good option if you want to skip using a webform tool too - it's a multistep workflow automation tool and can serve html pages as a step (so a form) - would require a bit of low-code since you'd have to create the html for the form. They do have Typeform + Jotform triggers however.
