Does removing attribution constitute a GPLv3 violation?

Yes. Removing attribution violates the license.
https://github.com/stefansundin/2fa-qr/blob/gh-pages/LICENSE states:
"Permissions of this strong copyleft license are conditioned on making available complete source code of licensed works and modifications, which include larger works using a licensed work, under the same license. Copyright and license notices must be preserved. Contributors provide an express grant of patent rights."
By removing attribution, the "Copyright and license notices must be preserved." clause is being violated.

Tip: Securing your AWS root account
I've used this one https://github.com/stefansundin/2fa-qr with great success. Good if you have to recreate the QR code from a previously recorded secret. (It's easier to scan the QR code, than to transfer the secret from print to the authenticator.)

One thing I've found very very useful when I just want to send a short text or a link from PC to phone is to use a QR code generator. I've used this so often that I've installed a firefox plugin to generate QR codes offline: https://github.com/rugk/offline-qr-code

On my Linux-/Windows-PC I use firefoxplugin QR & Barcode Generator. The QR-Code-Window shows the current browser URL as QR-Code but you can type in any text that immediately converted to QR-Code.
