The main drawback that steers most people away is wanting to access credentials on more than one device without having to manually move the database file. This is why password managers like LastPass, 1Password, DashLane, or NordPass are so popular. But you have yo trust the provider, their encryption, and that their security is up to industry standards without someone mishandling the servers that hold your data. To avoid this requirement without having to upload my KeePass to a cloud or migrate databases manually, I used a piece of FOSS called Syncthing to set up a secure, continuous file sync between my phone and computer. This ensures that whenever I use my KeePass databases on my phone, any updates are immediately applied to the copy on my desktop (and vice versa). This allows you all the security of KeePass' offline, locally stored functionality, while also allowing you to access it across multiple devices without any additional work on your end (just like the aforementioned premium providers which charge you $$$ for this service).
I personally use KeePass as it functions on both mobile and desktop as a Password Manager, TOPT, and can handle uploading attachments (like recovery codes for your accounts) directly to each corresponding entry.
