Do people still use ranvier MUD
Looking at the commit frequency on their GitHub, it looks like it hasn't been updated in a while. I'm not sure if anyone's working on it behind the scenes and just hasn't pushed to the main repository, but it's not a good sign that the last public push was 2 years ago.

Then that depends on the MUD engine you're planning to use. Evennia is in python, which may help since you're familiar with it, although it is a pretty large and complicated beast. Ranvier is a pretty simple and streamlined MUD engine that is written in Javascript and likely a great place to start if you're not an old MUD player who wants to recreate their old SMAUG or Circle favorite mud. It can be installed via node.js even. Both Ranvier and Evennia (I think) allow you to build the rooms interactively within a running mud (not 100% sure on that)

Indeed! Another one I forgot to mention is Ranvier in javascript and node.js which I think also allows websockets to communicate in ways other than telnet.

I can't speak for in-game stuff, but I encourage anyone who wants to to try making a visual novel! It's way easier than you'd think it is. The only tools I really used were Ren'Py (the engine), CSP and Paint.net (for the art), and KKT (for the models).

Otome game companies to work for
If you want full creative control, I would suggest going your own way and making indie games. The visual novel engine, Ren'py, is free to use and fairly easy to learn. They have a lot of helpful resources including Lemmasoft Forums, which is a great place to ask questions and find talent such as artists (we found our sprite/CG artist, BG artist, and GUI artist here, along with one of our voice actors).
