How does pmOS make those .zip files that you can flash in TWRP?
As for what I did with those zip files? I made the equivalent for another Linux on Android devices distro. The implementation probably won't suit your tastes, but it's something else to look at to figure out what can be done.

Logs sometimes show my VPNs IP, instead of device name
Open an issue in https://github.com/nextdns/windows/issues, I don't use the app but that would happen if the app somehow sends the request without your identifier, which is definitely a bug.

Possible bug with the Windows app?
Yes there is a bug. Easy workaround is to restart NextDNSService in the task manager > Services. It is somehow reported here: https://github.com/nextdns/windows/issues. A fix would be welcome.

Windows Defender thinks NextDNSService.exe is a virus
The NextDNS icon showed up on my desktop sometime today, indicating that an update had been installed. However, the last commit to the Windows client was 12 days ago. Could it be that the update just got pushed to the public?

Stopped using the Windows client due to it blocking Windows from knowing it's online.

For the official client, that issue is still open, unfortunately for YogaDNS I can't find any information if it actually supports silent install.
