I've found the nextcloud forms app to be good. Many free nextcloud providers for small instances.

Nextcloud Forms does not offer yet the ability to upload files directly from the form : https://github.com/nextcloud/forms/issues/42

Cryptpad.fr has forms, but I don't think you can upload attachments. Nextcloud has a forms app, support for attachments should land in the next version.
