Change rom language
So I was wrong about that, RA language was indeed supposed to change the DS language with melonDS! But it wasn't working properly and I just fixed it: https://github.com/libretro/melonDS/pull/170 , so it should be fixed in the next version of the core (it might take a few hours before it's available from the online updater).

MelonDS doesn't save option configuration on exit
It's because it crashes on exit or close content. The crash is already known: https://github.com/libretro/melonDS/issues/98

Problems with desmume core.
No, it's been updated pretty frequently after a new dev took it over. A bunch of core options were added months ago to bring it closer to Desmume feature wise too. But there are some crashing issues after the latest 0.9.1 update that haven't been fixed yet: https://github.com/libretro/melonDS/issues/98

Core options not saving (Android)
Best workaround I found is what I did for the OP, create the .opt file on a device where it works (although for melonDS it's not that simple...), change the values and put the modified file on your other device. Clearly not ideal nor user friendly, but I can't think of anything else until the crashes on close are fixed.
