Once Apple's Stable Diffusion package does support img2img, someone could build an app that allows you to use your Apple Pencil to do a drawing, which would, under the hood, convert that drawing to a CGImage, and use that as the source image for img2img. I'm not calling it trivial, but once the support is there in Apple's Stable Diffusion package, it is pretty trivial for a developer to wire that all up (I've been tracking this sample project from a really great developer who has been updating the project every time Apple adds a new feature; I'd bet he'll do so when img2img support is added, too).
