SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
1) you need to reflash the screen firmware with the Marlin 2.0 version of it, found here. You just copy all of it into an SD card, and put it in the screen's SD card slot.
You can find the source code for it here. I believe it is still an ATMega2560 chip. If its not, it won't flash, so it won't break anything.
