SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
I've been using WG Easy for a few months. Very easy to set up, integrated easily with my Nginx reverse proxy, and hasn't caused my any hassles. Generates QR codes for connections, too!
Firezone is pretty good https://github.com/firezone/firezone. Since I’m building it, please let me know if you run into issues!
