Using a passive backdoor like this or Prism you can send an ICMP and have the target callback to some device inside the network that you are pivoting through. As you stated, depending on the network and intermediate devices, that may not be the right approach for the job.

Still, it could be adapted though for broader use cases. For example, instead of sniffing ICMP, you could use a raw socket to capture all IP traffic, or specific layer 4 protocols and implement port knocking in a manner similar to cd00r.c.
