As for the template, you just circle the current date. This makes it flexible if you need multiple pages for the same date or end up skipping days. The labeled lines on the left hand side correspond to the 24 hours in a day (one row of boxes per half hour). I put my list of todos in the upper right hand corner, my daily schedule/what I do with my time down the left side, and then whatever extra notes pop up in the leftover space.
