SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
It's not the easiest solution, but for this kind of customization I'd actually recommend an entirely different solution. Using pynetworktables2js (https://github.com/robotpy/pynetworktables2js) lets you change the theming via CSS. There is apparently a shuffleboard-like dashboard built atop pynetworktables2js (https://shuffleboard2js.readthedocs.io/en/latest/index.html), but I haven't tried it yet.
