Recent core contributor to USWDS (https://designsystem.digital.gov/) here. IMHO, the design system is one of the better ones out there for web and application development. It falls a bit short, supporting the numerous JS frameworks, but most design systems do.

Login.gov is pretty good. Source code viewable here.
That depends on the country. The design system for the French government is pretty good, up to date on the best practices, and with a strong focus on accessibility (and we need to pass an accessibility audit anyway to get a .gouv.fr subdomain) - though I agree that everything is on a too tight a deadline with understaffed teams, which is frustrating.
USWDS can take care of most if not all of the 508, IDEA, and accessibility from the start. I've enjoyed using it on top of Eleventy framework for a few sites.
www.gov.uk is one site (although with multiple different applications responsible for different parts of it) ran by the Government Digital Service, part of the Cabinet Office.
And thankfully, it's sparked a trend all across government.
