WSUS needs monthly optimization via some sort of cleanup script. The popular one went closed source a few years ago and paid only but there are a few free replacements available now. I don't know the best one but the first one I found was https://github.com/awarre/Optimize-WsusServer

WSUS Optimization Script is a comprehensive Windows Server Update Services cleanup, optimization, maintenance and configuration PowerShell script. HanSolo71 explains, "One of the big things you can do to help WSUS run well is optimize settings, DB configurations and the indexes.
