Why aren't code actions working for me with the 0.5 beta LSP?
I've also had some issues with code actions in JS using typescript language server, in some situations. Check this chunk of my configuration, it may help you https://github.com/kristijanhusak/neovim-config/blob/bleeding-edge/nvim/lua/partials/lsp.lua#L159-L172
