It's a hidden Summoner Icon, you literally can't buy it or get it via chest the only way to get it is via External Programm

I found an app called ChallengesAreEvil on github which is supposed to remove the showcased tokens on your profile.

You can also change your skin to any even if you don’t own it (only you see it) but for obvious reasons I can’t link that. I remember seeing a design flaw that no longer lets you remove all challenge tokens from your profile since they added the ability to choose the order of the tokens months ago, tried it now to see if it's fixed and it's STILL there. I then had to download a third party app I found on github (here) just to remove tokens from my profile. Man I love Riot Games.
