Stars - the number of stars that a project has on GitHub. Growth - month over month growth in stars.
Activity is a relative number indicating how actively a project is being developed. Recent commits have higher weight than older ones.
For example, an activity of 9.0 indicates that a project is amongst the top 10% of the most actively developed projects that we are tracking.

Task notes?
You can do this with themes. There is a theme modification so that anything you type between %% can be notes or comments about the task. When you leave the task it will "hide" the comments. The easiest thing to do is to download one of the user-created themes. I use jgclark, his toothpaste and tooth-bleach themes have the comment capability implemented. I am not entirely sure what you mean by "universal format for themes", but there I personally love Base16 convention with its recommendation for styling. This is what I ended up (re)implementing for Neovim: mini.base16. It's been around, so most of instruments should have support for this.
