You'd do something like this using OSC for avatars. You can find the documentation for that here. It's unlikely, however, that there is a easy pre-made solution laying around for your specific heart rate sensor (there's a ton of them out there, and each one is going to have a different way to interface with it, if they provide one at all -- CooSpo looks like they partner with specific apps and don't publish any API docs anywhere I can see with a quick google, so someone would need to reverse engineer it a little bit). After you're able to read heart rate values from the device, you'd connect to VRChat using OSC to drive an animator parameter on your avatar. You'd use that animator parameter to drive a display on your (custom) avatar. I see a GitHub with a rather simple script that can read heart rate values from some CooSpo chest strap, and it looks relatively simple, so I'm inclined to think it wouldn't be terribly difficult for your device.
