SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
The next alternative is [Mercury](https://github.com/mljar/mercury) framework. It turns Jupyter notebooks into interactive web apps. You just add YAML config in the first cell and the interactive widgets (textbox, sliders) will be added to the notebook. You can share the notebook with others. Your users can tweak widgets and execute the notebook (and download result as single HTML file). What is more, you can hide notebook's code if you want. The framework is open source, but has also commercial additional features like authentication. It's [website](https://mljar.com/mercury).
