To fully self-host a route planner (e.g. a "Google Maps" equivalent) you need GraphHopper for the routing and two other parts: you also need some visualization aka "maps" (e.g. with OpenMapTiles) and you need the address search (e.g. with Photon).

This web app is not (yet) as polished as Google Maps app but there are many projects that use the routing functionality of GraphHopper and offer "complete" apps like outdoor apps (komoot, trailrouter, ride with GPS, Locus Map, plotaroute, Kurviger) or other services (delivery hero, deutsche bahn, GLS, ...)
GraphHopper offers you the path finding functionality ("A to B routing") that you probably know from Google Maps and uses data from the incredible OpenStreetMap project (see also /r/openstreetmap).
