Prometheus paired with Blackbox exporter is the fastest way to do this. Alertmanager is bundled with Prometheus for pushing alert messages. Incredibly easy to setup, the worst part is yaml.
Sounds like you need a monitoring system. I'd suggest PRTG if you can afford it, Zabbix if you can't. If you're only looking to monitor a couple ports, the free version of PRTG would work.
