Wiki Software for VTT
If you really wanna self-host it, you could install a regular Wiki software like Oddmuse. Alex Schroeder wrote and uses it for his RPG stuff, and hosts a CampaignWiki where anyone can publish their campaigns. And of course there's also MediaWiki, like used by Wikipedia or the [Wiki Aventurica](https://de.wiki-aventurica.de/].

Wiki Software for VTT
Now, after note-taking apps, wikis, let's get to VTTs. They're basically there for three things: Battlemaps, rules/dice and note-keeping/journals. Foundry VTT (the only self-hosted one) is quite nice in all regards. You could even keep your notes locally and sync them, although that gets a bit technical. If you're doing DSA5, there are even neat official character sheets and dice rolling macros available.
