Running a private mail server for six years, easy peasy
I’ve been using this https://github.com/r-raymond/nixos-mailserver for 4 years for my personal mail and I haven’t had a single issue in that time. I think it takes me about the same amount of time as you to maintain but I also have a next cloud server running on the same machine.

Will any emacs package manager let me audit packages before installing them?
You might want to use nix (the package manager from NixOS) with flakes or niv (to pin your inputs), using home-manager to configure emacs and maybe use rycee's emacs-init module.

You can use the Nix tooling to show the diff between two derivations' closure.
I have a script which I use [1] after updating my Flakes to do just that : know what has been upgraded.
