Is it safe to have insertion with prepared statements, and no security for output?
For output, escape it. Basic usage, use htmlspecialchars or a library so you can escape data based on context ([Aura HTML is one example(https://github.com/auraphp/Aura.Html), or Twig for a full template engine).
