What is the best configuration for large player counts?
For 1.12.2 try using Akarin. Its a super optimised, (mostly "multicore") jar. And it even works with bukkit plugins!

Can I read player health with code while playing?
If you want, you could take a look at an older version of the same concept that I implemented to allow a Discord bot to send messages on a Minecraft server. While this plugin doesn't have the functionality you want you could easily adapt it for that purpose.
