I have access to info here: Addon which leads to a gitlab with limited info within its description. It mentions its use of Discord's Rich Presence API & pypresence API.

Real-time glucose updates on Discord
This script uses the API of the external LibreLinkUp app and sents some HTTP requests to retreive glucose data. It also uses qwertyquerty's Pypresence to update Discord's 'Currently playing status' in which the glucose level and other data is shown.

I have installed pypresence with pip3 install https://github.com/qwertyquerty/pypresence/archive/master.zip, as doing it through pip install pypresence only installs 3.0.0
