  Direct damage isn't tracked accurately when dealt to foes that the player hasn't directly damaged. It's computed using a guess about monster max hp (see here). And that guess is either the max hp of another foe of the same type the player previously hit (which will be wrong if max HP was modified for either foe), or derived from a max hp formula that that's incorrect for both NM and HM (see here).
