Ledger Nano S shows 0 BTC and ETH balances, no trx history either
1) You've got the correct seed phrase, and there's no passphrase / 25th word involved, but for some reason Ledger isn't deriving the same addresses anymore. You can use a tool like findeth.io (run offline only), put in your seed phrase and your Eth address, and see if it can find the right derivation path.
Did you try the findeth.io tool to see if it can derive your previous ETH address from your recovery phrase? That's the next thing I'd try. There are ways to do this for BTC as well but this is an easier first step just to see if wrong path derivation could be the problem.

Cannot find my original ETH address on Ledger, even though the seed phrase is correct
I've just tried using the same seed on another Ledger (Nano S) -- and had findeth.io search 2500 addresses in all standard path derivations.... and nothing.
That's what I thought -- but that's also what I feared, since I've already done this, and haven't arrived at the original address. I have also just tried findeth.io and searched over 5000 addresses in all the standard derivation paths, with no success.

Ledger Nano S firmware update - all funds lost
I searched through https://findeth.io/ (all derivation paths), no luck :-( Looks like these are absolutely new addresses. Not the ones I used in 2018.
