You think this is an exception? Whenever you are bored take a look at the list of issues, both open and closed, and maybe that will help you understand how many things are and have been broken in the past. PoB is a great but this idea of yours that everything is perfectly tested before it is released has nothing to do with reality. The number of combinations of possible effects is limitless and PoE actively makes it hard or even nearly impossible to test these things with any degree of accuracy. PoB wouldn't exist if it everything had to be tested to the level of what you seem to think it is.
