It's ACE medical. Ever since the medical update anything with a passThrough value of 0.1 or lower in the hitpoints configuration for a gear item is basically invulnerable. It not only affects mods like RHS which use that value to represent NIJ level IV body armor, but also some of the heavily armored vanilla content like the "GA Carrier GL Rig" & "Carrier Special Rig." It's been reported to ACE for more than a year and nothing has been done to address it, so unfortunately the only way to fix it for the time being is either not to use ACE medical or just avoid the content that's affected by it...
