I had a PR to put this into core a long time ago, https://github.com/neovim/neovim/pull/13654, feel free to take anything useful from that.
Dartls supports it https://github.com/dart-lang/sdk/issues/44682, and the dart neovim plugin by akinsho offers support for documentcolor (although just for dartls i think). cssls also supports it.
