Not self-hosted in the traditional web app sense but a command line app. Pandoc can covert between doc, pdf and odt. However how closely the formatting is preserved depends on the complexity of the formatting and indeed how formatting was applied in the original document. AFAIK no converter is 100% accurate 100% of the time.
The only solution I know is https://github.com/zelon88/HRConvert2 In my experience, it works okay, not well but okay. But only tried it for a few minutes.
