They actually have a pattern for larger projects in the docs. I come from asp.net core at my day job and built a few sites using django. Stumbled across fastAPI when looking for lightweight/smaller frameworks for quicker dev time. I looked at Flask and a few others but not really familiar with their patterns. As you said, Flask and FastAPI seem to have a lot of similarity/overlap.
