SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
So basically look at the current option landscape and try to figure out what's like The Art of Computer Programming (effortlessly human-readable plaintext, todo.txt, Markdown files) and what's like Macromedia Flash (most anything where the workflow depends on proprietary programs or paid websites, closed formats, things where you can sort of export the data but it's a messy, rat's nest of JSON and you're expected to use the proprietary app to view it), and don't get invested in the Macromedia Flashes even when it's 1999 and they're really big right now.
I did, I actually tried a few when I made the switch: Notable, Standard Notes and Obsidian.
