SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
I am going to list this as separate because it is fairly unique. www.netlifycms.org is free, it saves the data as a file directly to the GitHub repo. On save the GitHub repo can trigger a build and update the site. The entire thing can be implemented on the front-end. If you couple this with Next, Nuxt or some other static site builder the website will be fast and can be hosted very cheap.
NOTE:
The number of mentions on this list indicates mentions on common posts plus user suggested alternatives.
Hence, a higher number means a more popular project.
