It's 'powered by' https://github.com/cstate/cstate For active monitoring I am using https://uptimerobot.com/ (currently free) so I get an SMS/email when the site goes down.
For logging errors from my app I am using https://rollbar.com/ (also currently free) and I may try to integrate rollbar with my status page ('Elevated error rates' kind of thing triggers an incident) at some point.
But...I don't want to burst your bubble too harshly, but the space your getting into is very crowded (and this is not everything): https://github.com/ivbeg/awesome-status-pages
I also think it needs to be running on different infrastructure than your app/site (so that if your hosting provider is down/unreachable, your status page will still be up). For example my app is hosted on fly.io while my status page is hosted by https://www.netlify.com/
