  • I'd like to suggest Overleaf again since it seems to be exactly what you need. If you don't want to pay for more than one collaborator, you can actually host it yourself. You can add users and administrators as you like (you can also add an SMTP server to allow sending email invitations to collaborators).

  • SurveyJS

Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.

  • I had no idea either! Give this guide a shot. It's for their Overleaf Toolkit, which is designed to get up and running with minimal complexity. If you need any help, feel free to PM me. I'll do my best to help.
