Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
There is also DIY versions, but I don’t know how technically inclined you are. I personally use https://github.com/vrtmrz/obsidian-livesync with a selfhosted CouchDB, but you can also use a free IBM Cloudant instance. Instructions on how to set it up are in the Github. It works really well and sync blazingly fast. I personally use it on Windows, Linux, macOS, iOS and it works very well everywhere.
