SurveyJS
Open-Source JSON Form Builder to Create Dynamic Forms Right in Your App. With SurveyJS form UI libraries, you can build and style forms in a fully-integrated drag & drop form builder, render them in your JS app, and store form submission data in any backend, inc. PHP, ASP.NET Core, and Node.js.
Personally, as a huge fan of Supabase, I would go with a CMS with a good free tier like for this kind of app. I would recommend Sanity CMS because they have a well made and highly customizable "portable text editor" to help any author easily write articles with embedded things without needing to write code themself. If you need to store any other data besides what you mentioned, you can use Supabase alongside Sanity for managing the blog content.
