Vessel VS docker-php-extension-installer

It wouldn't be part of the Dockerfile but you could use the "start-container" pattern as seen here: https://github.com/shipping-docker/vessel/tree/master/docker-files/docker/app

Some projects still using Vessel but anything new I'm now using Sail with a serve script that figures out which command to run. It also has a few others it checks for like WordPress and Hugo and falls back to a php -S of the current directory

Our "fix" was eventually move away to containers (local dev docker, production k8s) and never look back. Essentially https://github.com/mlocati/docker-php-extension-installer fills the void perfectly.

This helped me deal w/ installing missing extensions.
