PDF to doc and odt
Load up a Linux system and install pandoc on it. RTFM and convert anything to any other format. Doesn't do the OCR portion of your want though; that'd be a different tool.
Not self-hosted in the traditional web app sense but a command line app. Pandoc can covert between doc, pdf and odt. However how closely the formatting is preserved depends on the complexity of the formatting and indeed how formatting was applied in the original document. AFAIK no converter is 100% accurate 100% of the time.
