Nextcloud performance improvement
Next thing would be to install the "preview generator" app, as it will generate the preview images in the background and when you scroll through your photos it will use the smaller preivew images. https://apps.nextcloud.com/apps/previewgenerator
You should configure the previewgenerator app before the first run, check for this the github page. FAQ: "I don't want to generate all the preview sizes" would be important. https://github.com/nextcloud/previewgenerator

Self-Hosted "Cloud" suggestion
To make it a better, you could run the app Preview generator: it will generate a thumbnail at the size you want (e.g. 300x300) for each photo. At least, the thumbnails would show quickly. It can take some time and CPU, so preferably run it at night.

Runs just like the other preview backends, if you want it to run on a schedule use previewgenerator

Tip: For those with performance issues on Nextcloud
I'm using Preview-Generator on my private server. It generates all Thumbnails initially and all new afterwards with Cron job. https://apps.nextcloud.com/apps/previewgenerator

Anyone else abandon Nextcloud (Thinking of going to FileRun)
One other thought that I've only seen tangentially mentioned - have you tried pre-generating previews (https://apps.nextcloud.com/apps/previewgenerator). Perhaps the default on-the-fly method is falling down for some reason.
