    On OSX, try this. There is also the US Extended keyboard: to type the accented letters ĉ, ĝ, ĥ, ĵ or ŝ, press option-6, and then c, g, h, j or s. This also works for the capital letters. For ŭ: press option-B, and then u. This also works for Ŭ.

    The main Dvorak layout has been adapted for other languages: there are German, Spanish, Nordic and other variants with accents and perhaps some letter swap. Anecdotally, some people seem okay with that. Notably, this repo has English, Czech and other locales for Dvorak and according to the author, it's sufficient based on running the numbers for hand alternation and such metrics.

    There are at least three alternatives off the top of my head: CShack, UCW (the website is dead, but the layout is included in Linux distros) or some international layout such as EurKEY (although it lacks combined letters with caron), and then customized versions of Dvorak or Colemak... and yes, I know a couple of people that use Compose.

    I'm not convinced that it matters in practice. If anything, I'd focus on convenient access to special symbols (like ULKL).
